Microneedling Before and After: What to Realistically Expect
Last Updated: March 2026 · Reviewed by Tiffany Wang, PA-C · Skin Select, Brentwood Los Angeles
Microneedling creates controlled micro-injuries across the skin's dermis, triggering the body's wound-healing response and stimulating new collagen and elastin production. Most clients see visible improvement in skin texture, pore size, and fine lines after two to three sessions spaced four to six weeks apart. Results develop gradually over four to twelve weeks per treatment as new collagen matures and reorganizes below the surface.

What Microneedling Actually Does to Your Skin
A device equipped with fine needles creates thousands of microscopic channels in the skin at a controlled depth, anywhere from 0.5mm to 2.5mm depending on the treatment area and the concern being addressed. Those micro-injuries trigger a healing cascade, your body responds by producing new collagen and elastin to repair the channels, and that new tissue is denser and more organized than what existed before.
The depth matters because different concerns require different levels of penetration. Surface texture and pore size respond well to shallower settings. Acne scars involve structural changes deep in the dermis and typically require deeper passes and more sessions. Your provider sets the depth based on what your skin actually needs.

The Realistic Before & After Timeline
Mild redness, return to your day
Your skin will look red and feel tight, similar to a mild sunburn. Most clients return to normal activities the same day. Makeup is fine the following morning. The visible effects resolve quickly; what's working is happening below the surface.
Healing underway, invisible from outside
Some clients notice mild flaking as the skin turns over. Others see very little surface change. The inflammatory phase has concluded; collagen synthesis is accelerating below. This phase is invisible, but it's where the results are built.
First visible changes appear
Skin texture typically improves first, a smoothing quality that reads as more even and refined. Pores appear smaller as surrounding skin firms. Fine lines become less defined as newly produced collagen fills the dermis beneath them. According to a review in the Journal of Cutaneous and Aesthetic Surgery, microneedling produces significant improvement in skin texture and acne scarring, with new collagen formation confirmed on biopsy specimens (Doddaballapur, 2009).
Results continue building
Collagen remodeling continues through week twelve. The improvements seen at week four keep developing during this period. For clients on a three-session protocol, the second treatment is typically scheduled here, which extends and amplifies the collagen response rather than starting the process over.
What Improves, and What to Expect Per Concern
Skin Texture & Pores
Usually the most visible result after the first or second session. The skin's surface becomes more uniform, and pores appear tighter as surrounding collagen reorganizes. Consistent across all skin types and tones.
Acne Scars
Rolling and boxcar scars respond well to microneedling, but require patience. A 2015 meta-analysis in Dermatologic Surgery found meaningful improvement after three to six sessions (Fabbrocini et al.). Deeper, older scars often need the higher end of that range.
Fine Lines
Fine lines caused by skin thinning and collagen depletion respond well. Deep dynamic wrinkles from repeated muscle movement benefit less from microneedling alone. A provider will tell you clearly which category your lines fall into.
Under-Eye Area
Shallow-depth microneedling can improve crepey texture and early hollowing under the eyes. The skin is thin here, so collagen stimulation produces meaningful improvement even with conservative parameters.
RF Microneedling vs. Standard Microneedling
| Standard Microneedling | RF Microneedling (Potenza) | |
|---|---|---|
| Mechanism | Controlled mechanical injury triggers collagen response | Mechanical injury + radiofrequency thermal energy at needle tips |
| Best for | Texture, pores, fine lines, acne scars | All of the above + skin laxity, jawline definition, crepey texture |
| Laxity treatment | Limited benefit | Significant, collagen contracts immediately from heat |
| Adjustability | Needle depth per pass | Depth + RF energy level + pulse mode, calibrated per zone in one session |
| Recovery | Redness resolves same day | Redness resolves same day to 48 hours |
For texture improvement, acne scarring, and fine lines, both approaches produce comparable results. For clients with any meaningful degree of laxity, looseness along the jawline, crepey texture in the neck, or loss of definition under the eyes, RF microneedling produces more significant structural improvement because of the thermal mechanism.
Boosting your results: Both microneedling and RF microneedling pair well with biologics. PRP (platelet-rich plasma) applied after treatment delivers concentrated growth factors through the open microchannels, accelerating collagen output and improving outcomes for acne scars. Exosomes take this further, delivering cell-signaling molecules that support more intensive tissue repair. Your provider can advise whether adding either makes sense for your skin goals.
With RF microneedling, we're targeting both the dermal architecture and tissue firmness at the same time. For clients dealing with texture concerns and some laxity, combining both mechanisms in a single treatment gets a more complete result than addressing either one separately.
How Many Sessions Do You Need?
The spacing between sessions matters as much as the number. Sessions scheduled less than four weeks apart don't allow the collagen remodeling cycle to complete, you're adding inflammation before the previous round has delivered its full benefit. Four to six weeks between sessions is standard for a reason.
"The clients with the clearest before-and-after results are almost always the ones who completed a full series and came in for every session. One treatment is enough to understand how your skin responds. A series is what produces the result worth keeping."
- Have active acne breakouts, open wounds, or skin infections in the treatment area
- Have a history of cold sores (herpes simplex) — treatment can trigger an outbreak without prophylactic antiviral medication
- Are prone to keloid scarring
- Are pregnant or breastfeeding
- Are currently using isotretinoin (Accutane) or have done so in the past 6 months
Frequently Asked Questions
What does microneedling look like right after the treatment?
Immediately after a session, the skin appears red and feels tight, similar to a mild sunburn. The intensity varies with treatment depth and individual skin sensitivity. Most clients find the redness resolves within a few hours to a full day. By the following morning, the skin looks close to normal, with some residual pinkness that makeup can cover.
How long does it take to see microneedling results?
Most clients notice a visible improvement in skin texture and tone three to six weeks after their first session. The full result from a single treatment continues developing through week twelve as collagen matures and remodels. For acne scarring, meaningful change typically becomes apparent after the second or third session.
How many microneedling sessions are needed for acne scars?
Three to six sessions is the standard recommendation, with deeper structural scars typically requiring the higher end of that range. Rolling scars and boxcar scars respond well, but improvement builds gradually across sessions. Each session should be spaced four to six weeks apart to allow the collagen remodeling cycle to complete.
What's the difference between microneedling and RF microneedling results?
Standard microneedling stimulates collagen through controlled mechanical injury. RF microneedling adds radiofrequency energy through the needle tips, producing immediate collagen contraction alongside the wound-healing response. For texture and acne scarring, both approaches produce comparable results. For skin laxity, RF microneedling produces more significant tightening.
Is microneedling safe for darker skin tones?
Microneedling is well established as safe and effective across all skin types, including Fitzpatrick types IV through VI. Unlike laser treatments that use heat or light energy, microneedling's collagen-stimulating mechanism does not carry the post-inflammatory hyperpigmentation risk that makes some treatments unsuitable for darker skin. Skin Select treats all skin types.
How long do microneedling results last?
Results from a completed series typically last twelve to eighteen months before the natural aging process gradually reduces the collagen gains. Maintenance sessions once or twice a year can preserve the improvement. Results hold longer when paired with a consistent medical-grade skincare routine.
